May 6, 2013 ? Millions of seniors are turning down free money.
The Low Income Subsidy for Medicare Part D is a rare beast in economics research. The subsidy provides prescription drug coverage essentially free for low-income adults. That means it is what economists call a dominant option. For those who are eligible, there is no rational reason not to choose it. And yet, a new study shows that many eligible seniors do not take advantage of the program, despite outreach efforts by the Social Security Administration.
"We examined the role of seniors' cognitive abilities in explaining this puzzle," said J. Michael McWilliams assistant professor of health care policy and medicine at Harvard Medical School and Brigham and Women's Hospital and senior author of the paper. "If impaired abilities to access and process information are a root cause, simply providing seniors with more information is unlikely to help them make better choices."
McWilliams and his team analyzed data from the nationally representative Health and Retirement Study on seniors who were likely eligible for the Low Income Subsidy and found that seniors with lower cognition and lower basic math abilities were less likely to enroll in Medicare Part D, and less likely to apply for the subsidy if they did enroll in Part D. The study results will be published in JAMA Internal Medicine on May 6.
Can competition drive value?
In addition to raising important questions about how to best extend important benefits to a vulnerable population, the study raises questions about the extent to which consumer choices can be relied upon to reward value and drive competition in Medicare in general, the researchers said.
Medicare Part D provides seniors with a variety of plans to choose from for prescription drug benefits. The low-income subsidy reduces the cost of premiums and co-pays, and for the lowest income group the cost of benefits is entirely covered.
Those who qualify for Medicaid and are also in Medicare are automatically enrolled and get the subsidy without needing to take any action on their own. Other low-income adults must apply for the benefit, and estimates suggest that more than 2 million people who would qualify for a benefit fail to apply, according to a report from the Kaiser Family Foundation. The Harvard researchers focused on those who qualify but must apply for the subsidy.
"For those who are already enrolled in Medicare Part D plans and eligible for fully covered prescription drugs, this is not 'Here's thirty plans, pick the best one.' This is, 'Here's free money, do you want it?'" said lead author Ifedayo Kuye, a student at Harvard Medical School and Harvard Business School.
Even such a simple decision requires a high level of cognition, the researchers said. In order to meet the needs of this population, which includes many people with high medical needs, including chronic conditions, the design of the program probably needs to be changed, they said. One possible remedy the researchers suggest would be to make the subsidy an opt-out program for those who are eligible, so they would not need to take any extra steps to benefit from the program.
These findings have broader implications about how much reformers should rely on choice and competition in Medicare to drive down costs, the researchers said, because competition only works if consumers are choosing wisely and rewarding value in the marketplace.
The power of competition to drive value is an important component in many health care reform initiatives, including the proposal to transfer Medicare to a voucher system, and the health insurance exchanges that are being set up as part of the Affordable Care Act.
Seniors may need more help
"We may need the government or other agencies to do more on behalf of seniors and other health care consumers, both for their own well-being and if we want competition to be a force for driving up value in health care," Kuye said.
"Choice is generally a good thing, but options in health care can be very complex and consumers can have great difficulty identifying the best one," McWilliams said. "To improve decisions and enhance competition, the government and exchanges may need to play more active roles. Expecting the availability of choices and information for consumers alone to cure our health care system is unrealistic."

At some point -- we're not sure when -- Sony's naming scheme for laptops got confusing. There was the C series and the E series. The S, T, Z and even the F (RIP). Fortunately, Sony's been working to clear things up: its first Windows 8 products all had descriptive names like "Duo" or "Tap." Now, it's streamlining its mainstream notebooks too. The company just unveiled some redesigned models for back-to-school season, and they're all named 'Fit.' As the replacement to the entry-level E series and most of the T line, the Fit family includes both lower-end models (the Fit 14E / Fit 15E) and premium ones (the Fit 14 / Fit 15). They'll be available this month, starting at $550 and going all the way up to $2,210, depending on how many upgrades you check off during the configuration process.
Regardless, there are certain specs you'll find across the board - not just on the Fit machines, mind you, but on every Sony PC announced this season. These include NFC, backlit keyboards and webcams with Sony's Exmor R imaging tech for better low-light performance. They'll all have 1080p screens, too, with the exception of the 14-inch Fits, which start at 1,600 x 900. Both the Fit and Fit E will be offered with optional NVIDIA GT 740M GPUs (1GB or 2GB), touchscreens, optical drives and a choice of Core i3, i5 and i7 processors (Ivy Bridge for now). Ultimately, then, the difference between the two comes down to build quality: the Fit E is made of plastic, whereas the Fit has a thinner aluminum chassis, with the fans hidden above the keyboard, and a lid that covers the hinge. Also, the Fit will be offered with SSDs, with hybrid drives being the base option; the Fit E comes standard with a regular HDD and can be upgraded to a hybrid drive. The FBI has added the first woman to its ?Most Wanted Terrorists? list, Joanne Chesimard, who is currently living in Cuba under the name Assata Shakur.
Ms. Chesimard/Shakur is a fugitive member of a black militant group convicted of murdering a New Jersey state trooper by the side of a roadway on May 2, 1973. Sentenced to prison in 1977, she escaped in November 1979. Eventually she made her way to Cuba, where the Castro regime has sheltered her ever since, claiming she is a victim of racial persecution.
Cuba has declined repeated US requests to extradite her. On Thursday, the FBI and New Jersey law enforcement officials announced that they have doubled the reward for her capture and return to $2 million.
?She continues to flaunt her freedom in the face of this horrific crime," said New Jersey State Police Superintendent Col. Rick Fuentes at a news conference.

The FBI ?Most Wanted Terrorist? list is an offshoot of the venerable ?Most Wanted? list, and something of a younger brother. It was first drawn up and published in late 2001 after the 9/11 attacks. At that time it contained the names of 22 men, mostly Middle Eastern, who had been indicted by federal grand juries on charges connected to terrorist attacks ranging from the 1993 World Trade Center bombing to the 1996 attack on a US military residential tower in Khobar, Saudi Arabia. Osama bin Laden was No. 1. He was the only person on the 2001 terrorist list who was already a ?Most Wanted? ? in his case, due to his involvement in the 1998 bombings of US embassies in Africa.
Alleged terrorists remain on the list ?until such time as the charges are dropped or when credible physical evidence is obtained, which proves with 100 percent accuracy, that they are deceased,? says the FBI.
Needless to say, Mr. bin Laden is no longer listed.

Today the list has 32 names. The majority are Middle Easterners, such as Ayman al-Zawahiri, the former bin Laden associate and current listed head of Al Qaeda.
The first domestic terrorist added to the list, according to the FBI, was Daniel Andreas San Diego, in 2009. He is an alleged environmental extremist ? the FBI says he has ties to ?animal rights extremist groups? ? wanted for possible involvement in two 2003 bombings in the San Francisco area.
As for Joanne Chesimard, the FBI says she was a prominent member of the Black Liberation Army, which from the late 1960s through the 1970s was responsible for the killings of more than a dozen law enforcement officers.
On May 2, 1973, state police stopped a car in which Chesimard and two associates were riding on the New Jersey Turnpike on grounds that it had a broken taillight. At the time, Chesimard was wanted for possible involvement in several felonies, including bank robbery.
?Chesimard and her accomplices opened fire on the troopers. One trooper was wounded and the other was shot and killed execution-style at point-blank range,? reads the FBI?s account of what happened.
One of Chesimard?s companions was killed in the firefight. The other was convicted and remains in jail.
Not everyone agrees that Chesimard belongs on the terrorist list. They note that, among other things, the FBI and other law enforcement agencies used both legal and illegal means to watch and counter left-wing groups in the 1960s and ?70s.
Nor is there any evidence that Chesimard actually fired the shots that took the life of New Jersey state trooper Werner Foerster, said Rutgers University criminal justice professor Lennox Hinds, who was one of her defense attorneys. (Under New Jersey law, an accomplice can also be convicted of first-degree murder.)
?I believe that we have to look at this in the context of what has just happened in Boston,? Mr. Hinds told the self-described progressive news show "Democracy Now!" on Friday. ?I think that with the massacre that occurred there, the FBI and the state police are attempting to inflame the public opinion to characterize her as a terrorist, because the acts that she was convicted of have nothing to do with terrorism.?
The FBI, for its part, alleges that Chesimard took an active role in the firefight, including firing the first shot.
?This crime was always considered an act of domestic terrorism,? said Mike Rinaldi, a lieutenant with the New Jersey State Police and a member of the FBI Joint Terrorism Task Force in Newark, at Thursday?s press conference.

CAMARILLO, Calif. (AP) ? A big cool-down in weather calmed a huge wildfire burning in Southern California coastal mountains Saturday, and firefighters worked to cut miles of containment lines while conditions were favorable.
High winds and withering hot, dry air were replaced by the normal flow of damp air off the Pacific, significantly reducing fire activity.
"The fire isn't really running and gunning," said Tom Kruschke, a Ventura County Fire Department spokesman.
The 43-square-mile blaze at the western end of the Santa Monica Mountains was 56 percent surrounded.
The humidity level rose so much that an overnight effort to burn away fuel at one section of the fire did not work well, Kruschke said.
Despite the favorable conditions, evacuation orders remained in place for residences in several areas.
Nearly 2,000 firefighters using engines, bulldozers and aircraft worked to corral the blaze.
Firefighting efforts were focused on the fire's east side, rugged canyons that are a mix of public and private lands, Kruschke said.
The National Weather Service said an approaching low pressure system would bring a 20 percent chance of showers Sunday afternoon, with the likelihood increasing into the night and on Monday.
"Anything we get is going to help us," Kruschke said.
The change in the weather was also expected to bring gusty winds to some parts of Southern California, but well away from the fire area.
Despite its size and speed of growth, the fire that broke out Thursday and quickly moved through neighborhoods of Camarillo Springs and Thousand Oaks has caused damage to just 15 homes, though it has threatened thousands.
The fire also swept through Point Mugu State Park, a hiking and camping area that sprawls between those communities and the ocean. Park district Superintendent Craig Sap told the Ventura County Star (http://bit.ly/18CKxY4) that two old, unused ranch-style homes in the backcountry burned. Restrooms and campgrounds also were damaged. Sap estimated repairs would cost $225,000.
The only injuries as of Saturday were a civilian and a firefighter involved in a traffic accident away from the fire.
Residents were grateful so many homes were spared.
"It came pretty close. All of these houses ? these firemen did a tremendous job. Very, very thankful for them," Shayne Poindexter said. Flames came within 30 feet of the house he was building.
On Friday, the wildfire reached the ocean, jumped Pacific Coast Highway and burned a Navy base rifle range on the beach at Point Mugu. When winds reversed direction from offshore to onshore, the fire stormed back up canyons toward inland neighborhoods.
The blaze is one of more than 680 wildfires in the state so far this year ? about 200 more than average.
East of Los Angeles in Riverside County, a new fire that broke out Saturday afternoon burned 650 acres of wilderness south of Banning. It was 20 percent contained. Banning has been flanked by a nearly 5-square-mile fire to the north since Wednesday. Firefighters contained 85 percent of that fire, which destroyed one home shortly after it broke out.
In Northern California, a fire that has blackened more than 10 square miles of wilderness in Tehama County was a threat to 10 unoccupied summer homes near the community of Butte Meadows, according to the California Department of Forestry and Fire Protection.
Thunderstorms Saturday were expected to bring erratic winds but little rain to the area about 200 miles north of San Francisco.
Nearly 1,300 firefighters were on the lines and the blaze, which started Wednesday, was 20 percent contained.

But it?s often overlooked that the ultimate example of the old-school football coach was also perfectly fine with having gay players on his team.
Multiple players who played for Vince Lombardi, the legendary former Packers and Redskins coach, say that he knew some of his players were gay, and that not only did he not have a problem with it, but he went out of his way to make sure no one else on his team would make it a problem.
In 1969, Lombardi?s Redskins included a running back named Ray McDonald, who in 1968 had been arrested for having sex with another man in public. In the Lombardi biography When Pride Still Mattered, author David Maraniss writes that Lombardi told his assistants he wanted them to work with McDonald to help him make the team, ?And if I hear one of you people make reference to his manhood, you?ll be out of here before your ass hits the ground.?
Lombardi?s daughter Susan told Ian O?Connor of ESPNNewYork.com that her father would have been thrilled to have a player like Jason Collins, the NBA center who publicly revealed this week that he is gay.
?My father was way ahead of his time,? Susan Lombardi said. ?He was discriminated against as a dark-skinned Italian American when he was younger, when he felt he was passed up for coaching jobs that he deserved. He felt the pain of discrimination, and so he raised his family to accept everybody, no matter what color they were or whatever their sexual orientation was. I think it?s great what Jason Collins did, because it?s going to open a lot of doors for people. Without a doubt my father would?ve embraced him, and would?ve been very proud of him for coming out.?
Dave Kopay, the first former NFL player to come out, also played on those 1969 Redskins, and he says that while he never told Lombardi, he believes Lombardi knew not only that Kopay was gay, but that Kopay and another Redskins player, Jerry Smith, were in a romantic relationship.
?Lombardi protected and loved Jerry,? Kopay told O?Connor.
Lombardi?s brother Harold was gay, and when Harold died in July of 2011 he was survived by his partner of 41 years ? meaning their relationship began just before Vince died in September of 1970. As noted by Doug Farrar of Yahoo! Sports, Vince knew Harold was gay and didn?t just believe in ?tolerance? but believed strongly that discrimination against gay people was wrong, just as he was angered when he saw mistreatment of his black players, or discrimination against his fellow Italian-Americans.
If a coach who was considered old-fashioned even by the standards of the 1960s accepted gay players in his locker room, the idea that gay players couldn?t be accepted in an NFL locker room in 2013 is both silly and sad.

(Reuters) - International hit "Iron Man 3" flew into U.S. and Canadian theaters, kicking off Hollywood's summer blockbuster season with $15.6 million in ticket sales late Thursday and early Friday.
The superhero sequel from Walt Disney Co's Marvel studio is expected to finish the weekend with one of the biggest debuts in film history. Box office analysts predict the movie will bring in $150 million or more from Friday through Sunday, likely ranking in the top five all-time openings.
The opening night sales fell short of the $18.7 million earned a year ago by Disney's "The Avengers," which brought together Iron Man, The Hulk and other Marvel superheroes. "Avengers" went on to earn $207 million in its first three days, a record for the North American (U.S. and Canadian) market.
"Iron Man 3," starring Robert Downey Jr. as billionaire businessman Tony Stark, has pulled in $345.4 million in overseas markets since it debuted April 24, Disney said on Friday. The film set opening day records in several countries including China and Russia.
In "Iron Man 3," Stark fights to save the world from an extremist known as The Mandarin, played by Ben Kingsley.
